Overview: Sentara Health Plansis hiring a Care Coordinator/ Licensed Practical Nurse (LPN) for LOCERI/LTSS Waiver - Remote in Virginia Status: Full-time,permanent position (40 hours) Standard working hours: 8am to 5pm EST, M-F Location: This position is remote in Virginia only. Job responsibilities: - Support the Case Management team by reviewing completed LOCERI (annual assessments) to ensure accurate scoring and that all required elements are completed.
Maintain a productivity target of 12-15 reviews per day.
Education - Associate's degree in Nursing preferred.
- Bachelor's degree in Health & Human Services-related field or related experience in lieu of education accepted.
- Licensed Practical Nurse (LPN) or Bachelor's prepared health professional strongly preferred.
Certification/Licensure - LPN license in VA or Compact/Multistate preferred
Experience - 3 years Managed Care experience REQUIRED
- Case Management experience preferred
- Long-Term Services and Supports (LTSS) waiver experience preferred
