Responsibilities
Perform initial in-home pre-evaluation assessments of consumers.
Review consumer medical documentation and health referral forms.
Enter evaluation data into the UAS-NY online application.
Code UAS assessments correctly based on guidelines.
Document concerns, conflicting information, and outcomes.
Emphasize continuity of care to reduce fragmentation.
Discuss health care options and service vendor/waiver options with consumers.
Initiate enrollment into managed care health plans.
Stay updated on regulatory changes related to assessments.
Improve care coordination with interdisciplinary team members.
Review non-qualified assessments with the Quality Assurance Department.
Collect quality review data and document case notes.
Identify health promotion and illness prevention opportunities.
Maintain knowledge of community resources and network services.
Meet performance criteria and perform assigned duties.
Requirements
NYS RN Licensure required.
2 years of Homecare experience required.
Must be able to travel throughout Albany County.
