Work Environment
This full-time position offers the flexibility of a remote work setting, allowing you to manage your schedule while providing diligent care to our clients. As part of a multidisciplinary team based remotely, you will collaborate with psychiatrists, nurses, and other healthcare professionals to deliver holistic support and treatment tailored to each patient's requirements.
Key Responsibilities
Conduct comprehensive assessments to identify the biopsychosocial needs of clients, carefully evaluating their strengths, challenges, and treatment goals.
Develop individualized treatment plans aimed at improving patient functioning and symptom management, with progress monitored regularly to adapt strategies as necessary.
Provide various therapeutic modalities including individual, family, couple, and group psychotherapy, ensuring that all interventions are evidence-based and clinically suitable.
Engage in collaborative program planning with the psychiatry team, contributing your insights to enhance overall patient care and treatment strategies.
Document progress notes effectively using telehealth systems and electronic health records, with training provided to ensure you are comfortable with the technology.
Act as a case manager for assigned patients, coordinating care and resources to support their physical and emotional needs.
Patient Population / Client Focus
Our clients primarily consist of senior citizens residing in hospice settings, often dealing with complex emotional and psychological challenges associated with end-of-life care. Your role will involve not only directly supporting these patients but also working closely with their families, helping to navigate the grieving process and facilitating important conversations about care preferences and expectations.
Qualifications
Must possess an active Licensed Clinical Social Worker (LCSW) designation in Pennsylvania.
A Bachelor’s degree in Social Work or a related field is required.
Demonstrated capability in managing complex cases and providing psychotherapy is essential.
The ability to work autonomously, while also being an effective collaborator with other healthcare professionals.
Preferred Experience
Experience providing care to senior citizens, particularly within hospice or palliative care settings, is highly preferred.
While not mandatory, a Medicare provider number is advantageous for this role.
Previous experience using electronic health records systems is a plus, but comprehensive training will be provided.
A willingness to obtain professional liability insurance is required to ensure compliance and best practices in care delivery.
Compensation and Benefits
This position offers a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience. Additionally, you will be eligible for a comprehensive benefits package that includes:
