Love Bound Therapy is seeking a devoted and licensed Clinical Social Worker (LCSW) to join our dedicated team. Work Environment
This remote position enables our social workers to maintain flexibility while engaging closely with clients and interdisciplinary teams. Our work hours are typically Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening and weekend commitments based on client needs. Key Responsibilities
Conduct comprehensive assessments of seniors' psychosocial needs, integrating emotional, mental, and social factors into care planning.
Create individualized care plans in collaboration with seniors and their families, focusing on their specific preferences and goals.
Provide counseling and emotional support during life transitions, assisting clients with navigating challenges such as loss and health changes.
Advocate for clients' rights to access necessary community resources, ensuring they benefit from the support services available.
Coordinate with a multidisciplinary team—comprising nurses, caregivers, and other healthcare professionals—to enhance care delivery.
Perform regular check-ins via virtual visits to monitor client well-being, assess living conditions, and identify any potential safety issues.
Facilitate effective communication among family members and clients to resolve conflicts and ensure collective understanding regarding care strategies.
Educate clients and families about available community resources, support groups, and programs to improve their social engagement and overall quality of life.
Maintain meticulous documentation of client assessments, care plans, progress notes, and interactions, adhering to established ethical standards in social work.
Stay current with best practices and regulatory updates within the field of social work.
Patient Population / Client Focus

Qualifications
A Master’s degree in Social Work from an accredited institution.
Current and valid state licensure as a Clinical Social Worker (LCSW).
Minimum of two years of relevant experience in social work, with a preference for experience focused on geriatric populations or community service settings.
A strong understanding of social work principles, particularly in relation to aging and community resources.
Exceptional assessment, counseling, and problem-solving capabilities.
Demonstrated empathy, compassion, and ability to build rapport with clients and their families.

Preferred Experience
Experience working in a community health clinic or similar environment focused on senior care.

Compensation and Benefits
