Work Environment

Key Responsibilities
Conduct comprehensive psychosocial assessments through virtual consultations, identifying emotional, social, and financial needs of patients and their families.
Provide individual and group counseling tailored to address challenges including grief, adjustment issues, and coping strategies.
Facilitate crisis intervention and support services to alleviate patient distress and enhance emotional well-being.
Develop and implement effective discharge plans while connecting patients to community resources and services post-discharge.
Collaborate actively with interdisciplinary teams to improve patient outcomes, advocate for patient needs, and ensure continuity of care.
Maintain thorough and accurate documentation of all interactions and assessments in compliance with hospital policies and legal requirements.
Participate in hospital committees, offering insights and expertise on social work practices to inform organizational strategies.
Patient Population / Client Focus
The primary focus of this role is on patients with life-limiting illnesses and their families, navigating complex emotional landscapes during a pivotal time in their lives. We aim to provide compassionate support that honors their lifecycle while ensuring access to necessary resources, comfort, and emotional resilience. Qualifications
Master’s degree in Social Work from an accredited program.
Active state licensure as a Licensed Clinical Social Worker (LCSW) in Virginia.
Strong understanding of medical terminology and health systems, particularly in the hospice and palliative care settings.
Excellent communication and interpersonal skills to foster relationships with patients, families, and healthcare professionals.
Preferred Experience
While not mandatory, two years of experience in a healthcare or hospital setting is preferred. Familiarity with hospice care, mental health treatment modalities, and crisis intervention strategies will be advantageous in delivering effective and compassionate patient care.
