Key Responsibilities

Conduct thorough mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments to understand patients' needs.

Develop and implement individualized treatment plans in collaboration with patients, taking their cultural background and personal goals into account.

Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), across diverse settings including individual, couple, family, and group therapy.

Provide crisis intervention services to patients experiencing acute distress, ensuring they receive the necessary support and care.

Maintain accurate clinical documentation, including progress notes and treatment plans, to comply with clinic policies and state regulations.

Educate patients and their families about mental health conditions, empowering them with tools and resources for effective self-management.

Engage in ongoing collaboration with a multidisciplinary team to coordinate comprehensive patient care and ensure continuity of services.

Regularly assess treatment outcomes and make adjustments to care based on demonstrated effectiveness and best practices in mental health.

Continuously update your knowledge of current research and industry trends, integrating this information into your therapeutic practice.

Adhere to ethical guidelines and maintain strict confidentiality concerning patients’ information.

Patient Population / Client Focus Your primary focus will be on serving a diverse range of clients within the community health clinic setting. Patients may experience a variety of mental health challenges, from anxiety and depression to trauma-related disorders. Your ability to provide customized and culturally sensitive care will be essential in helping individuals achieve better mental health outcomes.

Qualifications

Master's degree in Social Work (MSW) is required.

Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

Demonstrated ability to work both independently and as part of a collaborative, multidisciplinary team.

Outstanding interpersonal and communication skills, particularly in establishing rapport with patients from varied cultural backgrounds.

Preferred Experience

Prior experience in a community health clinic or similar setting is a plus.

Familiarity with telehealth practices and platforms will be beneficial.

Compensation and Benefits The annual base salary for this position ranges from $115,000 to $130,000, depending on your experience, education, and skill level.
