Key Responsibilities
Conduct thorough and comprehensive assessments of patients to determine their psychological needs and challenges.
Develop and implement individualized treatment plans using evidence-based interventions tailored to the specific needs of your clients.
Maintain detailed and accurate clinical records, ensuring compliance with confidentiality standards and relevant regulations.
Collaborate with a multidisciplinary treatment team to provide holistic care, ensuring consistency and continuity in the support offered to patients and their families.
Educate clients and their families about coping strategies and available resources, empowering them to take an active role in their mental health journey.
Stay informed about the latest mental health research and best practices to continuously enhance your therapeutic techniques.
Patient Population / Client Focus
Your primary focus will be on patients receiving hospice care, many of whom are experiencing significant emotional and psychological challenges associated with terminal illness. You will work closely with individuals navigating complex grief and loss scenarios, providing critical support and therapeutic interventions that help enhance their quality of life during this sensitive time. The diverse backgrounds of our patients require an adaptive approach that addresses their unique circumstances and cultural perspectives.
Qualifications
A Master's degree in Social Work from an accredited institution is required.
A valid LCSW license in the state of Georgia is mandatory.
A minimum of two years of clinical experience, preferably within a behavioral health or hospice setting.
Proficiency in solution-focused psychotherapy interventions and familiarity with various therapeutic modalities.
Exceptional communication and interpersonal skills, allowing for effective collaboration with patients, families, and multidisciplinary team members.
Preferred Experience
Experience working in hospice or palliative care environments is highly valued.
