Compensation: $28-$34 per hour, depending on experience

Desert Bloom Family Medicine is seeking an experienced, dedicated Licensed Practical Nurse (LPN) to join our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) team.
We are looking for an individual with extensive, proven experience managing CCM and RPM patients, who can work independently and help us build a highly successful in-house program.
Prior CCM/RPM experience is REQUIRED. This is not an entry-level position.
Key Responsibilities
- Conduct monthly outbound calls to patients enrolled in CCM and RPM programs.
- Review remote monitoring data, including blood pressure, blood glucose, weight, and other clinical readings.
- Assess patient concerns within the LPN scope of practice and escalate findings to the supervising clinical team as appropriate.
- Coordinate care plans, medication-related concerns, follow-up appointments, and referrals with providers.
- Maintain accurate documentation of patient interactions and billable time.
- Support Medicare CCM/RPM compliance, documentation, and billing workflows.
- Identify opportunities to improve patient engagement, retention, and program performance.
- Assist in developing and expanding our in-house CCM/RPM services.
Qualifications
- Active, unrestricted Arizona LPN license.
- Extensive hands-on experience in CCM and RPM is required.
- Strong understanding of chronic disease management, Medicare requirements, and care coordination.
- Experience managing large patient populations and conducting monthly patient outreach.
- Excellent clinical judgment, communication, and organizational skills.
- Strong EHR documentation skills.
- Ability to work independently while collaborating with physicians and advanced practice providers.
- Bilingual English/Spanish is a plus.
