Position Overview Gather and Grow Counseling Center is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team. Key Responsibilities

Provide comprehensive psychotherapy services through virtual sessions, addressing a wide range of mental health issues affecting hospice patients and their families.

Utilize various psychological assessments and tools to perform thorough evaluations and diagnose mental health conditions, ensuring appropriate and effective treatment.

Develop personalized treatment plans that reflect each client’s unique needs, goals, and values, while integrating patient and family feedback into the therapeutic process.

Deliver crisis intervention services, including risk assessments and establishing necessary immediate care pathways, especially for suicidal ideations or severe emotional distress.

Collaborate with an interdisciplinary team of healthcare providers, including physicians and allied health professionals, to ensure seamless and coordinated care across all service aspects.

Maintain meticulous and confidential documentation of all client interactions and treatment plans in accordance with ethical guidelines and organizational protocols.

Contribute to the establishment of clinical policies and procedures that enhance service delivery and align with best practices in mental health care.

Participate actively in team meetings, case reviews, and ongoing training programs aimed at professional growth and service innovation.

Patient Population / Client Focus The ideal candidate will engage primarily with hospice patients and their families, guiding them through the emotional and psychological challenges of advanced illness and end-of-life transitions. You will work with individuals facing grief, anxiety, depression, and family dynamics, requiring sensitive support and tailored interventions. By fostering a therapeutic alliance, the LCSW plays a key role in promoting emotional healing during this critical phase of life.

Qualifications

A Master's degree in Social Work from an accredited institution and a current Licensed Clinical Social Worker (LCSW) license is required.

At least two years of relevant clinical experience in a mental health setting, particularly with individuals in crisis or those managing chronic illness.

Proficient in evidence-based interventions and therapeutic modalities that are suitable for a hospice care environment.

Proven ability to communicate effectively with diverse populations, demonstrating cultural competency and respect for individual backgrounds.

Strong critical thinking, organizational, and interpersonal skills, coupled with an ability to respond calmly and effectively in high-stress situations.

Preferred Experience

Previous experience working in hospice, palliative care, or similar environments.

Familiarity with telehealth technologies and methodologies relevant to remote client engagement.

Compensation and Benefits
