Key Responsibilities

Conduct detailed assessments of clients’ psychosocial needs, ensuring a holistic understanding of their emotional and social factors influencing their recovery.

Develop individualized treatment plans in collaboration with clients, focusing on their unique goals and preferences.

Provide ongoing counseling and support, helping clients cope with life challenges and transitions, especially in the context of recovery from substance use.

Advocate for clients by connecting them with essential community resources and support services to facilitate their recovery journey.

Monitor clients regularly through virtual check-ins, ensuring they are adhering to their treatment plans and identifying any emerging concerns or needs.

Collaborate with a diverse team of healthcare professionals to create a cohesive and effective care approach tailored to each client’s requirements.

Document all client interactions, assessments, and treatment plans using our electronic health record (EHR) system to maintain accurate and up-to-date records.

Stay informed about best practices in social work and substance use treatment to enhance your knowledge and skills continuously.

Patient Population / Client Focus The primary focus of this role is to support clients navigating detoxification and recovery from substance use. This population often presents with complex psychosocial challenges that require sensitivity, deep understanding, and tailored interventions. You will engage directly with clients seeking to construct a healthier and more fulfilling life while advocating for their rights and needs within the healthcare system.

Qualifications

A Master's degree in Social Work from an accredited institution.

Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

A minimum of two years of relevant experience in social work, particularly within substance use treatment or similar clinical settings.

Strong knowledge of social work principles, mental health issues, and substance use disorders.

Excellent communication, interpersonal, and advocacy skills that enhance collaboration with clients and treatment teams.

Proficiency in using electronic health record (EHR) systems for documentation and communication purposes.

Preferred Experience
