Provide personalized assessments to evaluate patients' mental health and social needs, ensuring that care plans are relevant and effective. Develop and implement therapeutic strategies that align with patients' goals, involving family members in the counseling process when appropriate. Coordinate with medical professionals and community agencies to secure essential services for patients, enhancing their overall quality of life. Document patient interactions promptly and accurately, maintaining thorough records in accordance with healthcare regulations. Facilitate educational workshops to guide patients, families, and staff on coping mechanisms for stress and trauma. Monitor ongoing patient progress, adapting treatment plans proactively and ensuring continuity of care. Engage in regular team meetings to collaborate with interdisciplinary team members, sharing insights that uphold the highest standards of patient care. Patient Population / Client Focus

This role focuses specifically on hospice patients, who often face complex emotional and psychological challenges. You will work with individuals experiencing terminal illnesses and their families, providing essential support during a critical time in their lives. Your expertise will play a vital role in helping these clients navigate their emotions and finding peace of mind as they approach end-of-life decisions. Qualifications

A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline is essential. Must possess a current independent license as a Licensed Clinical Social Worker (LCSW) in California. Excellent verbal and written communication skills, with an emphasis on accurate documentation. Strong proficiency in Microsoft Office and Electronic Health Record (EHR) systems, ensuring effective use of technology in patient management. Ability to work both independently and collaboratively in a remote team setting. Preferred Experience

Experience with Salesforce or similar case management systems is highly desirable. Prior work in hospice, palliative care, or similar settings will provide an edge in understanding the unique challenges faced by patients and families. Demonstrated knowledge of community resources available to support patients in hospice care. Compensation and Benefits

We offer a competitive salary ranging from USD 80,000 to 122,000 annually, commensurate with experience and qualifications.
