This position offers a remote work setting, allowing for flexibility while serving a diverse senior population. You will collaborate with interdisciplinary teams comprising healthcare professionals, caregivers, and family members to ensure the best outcomes for clients. Regular communication through virtual platforms ensures that team members remain connected and responsive to client needs, and scheduling flexibility is essential to accommodate client appointments and emergencies. Key Responsibilities

Conduct comprehensive assessments of clients' psychosocial needs, taking into account their emotional, mental, and social well-being. Develop tailored care plans in collaboration with seniors and their families, aligning with individual goals and preferences. Provide counseling and emotional support to clients, facilitating their adjustment to life transitions and challenges. Advocate for clients' rights within the community, ensuring access to essential resources and services. Collaborate with a multidisciplinary team to coordinate care and enhance the quality of support provided to clients. Perform regular check-ins and home visits to monitor client progress, living conditions, and overall safety. Facilitate healthy communication among family members contributing to the care of seniors. Educate clients and their families about community resources, including support groups and engagement programs, promoting social interaction and involvement. Maintain accurate documentation regarding client assessments, care plans, and progress notes, adhering to compliance and ethical standards. Engage in professional development and stay current on social work practices, policies, and regulations. Patient Population / Client Focus

This role focuses primarily on seniors and their families, providing holistic support to address the complexities of aging. You will work with clients facing various challenges, including mental health issues, social isolation, cognitive decline, and navigating healthcare systems. The well-being of seniors is at the forefront of your responsibilities, with the ultimate goal of ensuring a fulfilling quality of life for each individual. Qualifications

A Master’s degree in Social Work from an accredited institution is required. Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of Illinois. At least two years of professional experience in social work, particularly within community centers or geriatric settings. Strong understanding of social work principles, aging-related issues, and community resources. Exceptional assessment, communication, and counseling skills, with the ability to empathize and build rapport with seniors. Proficiency in using electronic health record (EHR) systems and technology to document client interactions and care plans. Preferred Experience

Experience working with geriatric populations or in community-based settings is strongly preferred. Candidates with familiarity with local resources and services for seniors will have an advantage in this role, as understanding the community landscape is crucial for effective advocacy and support. Compensation and Benefits

The salary for this position ranges from $60,000 to $100,000 per year, based on experience and qualifications.
