Key Responsibilities

Develop tailored treatment plans for clients by assessing their individual needs and goals, utilizing evidence-based therapeutic approaches.

Conduct individual therapy and group therapy sessions that promote mental wellness and recovery, all while ensuring the confidentiality and privacy of clients.

Engage in team-based meetings to discuss treatment progress, share insights, and collaborate on comprehensive care strategies for clients.

Perform thorough assessments to analyze client information, aiding in accurate diagnoses and facilitating appropriate treatment paths.

Lead and implement therapeutic activities, including group dynamics and recreational activities that foster engagement and support healing.

Employ a coaching model and adapt natural environment teaching methods to enhance therapeutic outcomes.

Patient Population / Client Focus The clients you will serve are individuals dealing with various mental illnesses and substance use issues, specifically within the unique context of hospice care. Many clients may be experiencing significant transitions, facing end-of-life issues, or dealing with advanced health conditions. Your role will be pivotal in offering not just therapeutic support, but also comfort and guidance during their journeys, helping to improve their quality of life by addressing emotional and psychological needs.

Qualifications

A valid and unrestricted license as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

A Master’s degree in Social Work, Counseling, or a related field.

Demonstrated strong organizational skills, prioritization abilities, and effective time management.

Willingness to engage in reflective practices and openness to feedback as part of continuous professional development.

At least three years of post-licensure experience in a mental health or related field.

Preferred Experience

Experience in hospice or palliative care settings is highly desirable.

Familiarity with telehealth platforms and techniques, as well as group therapy facilitation.

Proficiency in understanding and applying evidence-based therapeutic approaches tailored for clients with complex needs.
