Key Responsibilities

Provide evidence-based therapeutic interventions tailored to a diverse patient demographic dealing with mental health challenges.

Conduct comprehensive patient intakes to understand individual needs, establishing treatment goals collaboratively.

Facilitate both individual and group therapy sessions via video conferencing or telephone, adapting techniques to suit the telehealth format.

Perform mental health assessments and develop individualized treatment plans, regularly reviewing client progress and adapting as necessary.

Document all interactions, therapy sessions, and patient progress with precision in compliance with Federal, State, and organizational standards.

Provide timely referrals and connect clients with additional supportive resources as needed, ensuring a holistic approach to their care.

Patient Population / Client Focus Your primary focus will be on individuals within the hospice care spectrum—patients, their families, and caregivers—who require mental health support during challenging life transitions. Qualifications

A Master’s degree in Social Work with an active Licensure as a Licensed Independent Clinical Social Worker (LICSW).

A minimum of 2 years of professional experience in mental health counseling is required.

A minimum of 1 year of experience in a remote work environment, demonstrating effective communication and virtual engagement skills.

Proficiency with telehealth technology, comfortable navigating video conferencing tools and electronic health record systems.

Excellent organizational and documentation skills, ensuring adherence to clinical best practices and legal requirements.

Preferred Experience

Experience working in a hospice or palliative care setting is highly desirable.

Proven success in building therapeutic rapport and maintaining patient engagement through virtual platforms.
