This role allows for a flexible work life within a remote environment, enabling you to provide services to our patients through telehealth solutions or outpatient consultations. Conduct comprehensive psychosocial assessments to evaluate the needs, strengths, and challenges of patients and their families. Develop individualized treatment plans that take into account the unique circumstances and preferences of each patient, focusing on facilitating dignity and comfort. Provide psychotherapy and emotional support to patients and caregivers, guiding them through complex feelings around illness and end-of-life issues. Collaborate with interdisciplinary team members—including nurses, physicians, and chaplains—to ensure holistic patient care and adherence to best practices in hospice care. Document all interactions and interventions in patient records to maintain compliance with state regulations and organizational policies. Stay informed about current developments in mental health and hospice care, integrating evidence-based practices into your work. Patient Population / Client Focus

This position primarily serves patients in hospice care, often facing terminal illnesses. The focus is on individuals who may require extensive emotional and psychological support during their transition. You will engage with patients of diverse backgrounds and ages, addressing not only their psychological needs but also providing family support, ensuring a holistic approach to care. Qualifications

A Master’s Degree in Social Work (MSW) from an accredited institution. Possession of an active Licensed Clinical Social Worker (LCSW) certification is mandatory. A minimum of two years of clinical experience, preferably in hospice or palliative care settings. Strong interpersonal and communication skills, with an ability to enhance the therapeutic relationship with patients and families. Proficient in electronic health records and documentations, with a keen understanding of HIPAA regulations. Preferred Experience

Experience working in hospice or palliative care teams, focusing on end-of-life issues. Familiarity with grief counseling interventions and bereavement support. Demonstrated ability to work collaboratively in a multidisciplinary team environment. Compensation and Benefits

We offer a competitive annual salary ranging from USD 90,000 to 100,000, commensurate with experience and qualifications.
